Bids are hereby invited for the appointment of a service provider for the supply, delivery and construction of prefabricated mobile offices for the registration of birth in health facilities located in nine (9) provinces across the country, as and when required for the period of three (3) years [renewable up to two (2) years subject to satisfactory performance, availability of funds and reasonable commercial terms and conditions]. Scope: The successful bidder will be expected to execute and conduct the following tasks for/provide the following services/items to the Department: - Develop a project plan which covers the scope of work, business requirement and project deliverables; - Site preparations prior to erection of mobile structures including site clearance; - Preparation for architectural drawings; - Any other professional services relating to the prefabricated Mobile office environment; - Submit progress reports on the implementation of the project bi-weekly for consideration and verification; - Sign-off work completed in line with the specifications and approval from the client - Handover all documents pertaining to the project and provide certification of completion per site; - Provide certificates for the provision of maintenance of the prefabricated mobile offices to the client; - Design, supply, construct and deliver prefabricated mobile offices for the registration of birth in health facilities in nine (9) provinces, as and when required, for the period of three (3) years.
